The verdict returned whenever the rater cannot produce a trustworthy answer (LLM throws, times out, or returns unparseable output). Fail-closed: destructive, never auto-approved, and never attack or catastrophic either — a failure to assess must not manufacture a run-halting outcome any more than it may manufacture an approval.
