Usage copy for any malformed /approvals subcommand. It dispatches on the problem’s kind
rather than on which subcommand was typed, so a new subcommand’s problems are explained by adding
a case here and cannot fall through to another subcommand’s copy.
Nothing was changed is stated on every branch, and on the [[EXT-107]] ones it is the part that matters: these arrive from a command that would have REMOVED a refusal, and a user who mistyped the number has to know the gate is still where they left it.
